Beating yourself up won't help...
It sounds like waiting so long for a new quit date is giving yourself permission to keep smoking, but as Penitent has said, it's your quit you really do manage it...
There really are quitters who have a successful Quit the first time around, maybe not many, but I am one of them, so it can be done. And you can have a successful quit too. When you want to quit more than you want to smoke, you can do it!!
Please do some deep soul searching, and when you are ready we will be here...
