Start with very small goals centered around work and work from there increasing the size and importance in a very slow steady way.
Reaching those small goals and others thereafter will help you build some confidence and perhaps motivate you to acheive bigger goals.

My depression was once more debilitating, but following therapy (and lots of trial/error), I've become pretty diligent about doing what makes me feel better (exercise, volunteering, painting, meditation, seeing friends).
But I am still very anxious, depressed (scored 70-80 on initial test) and have difficulty concentrating and focusing.
Work is still a real challenge and would seem a good place to set some treatment goals. (I was a staff writer and quit my job because of depression, now I freelance.)
But I'm afraid to set goals around work because I'm already so anxious and feel like such a failure.
Any suggestions are greatly appreciated.
